What happened to the epistemic claim of fully understanding the mechanisms of disease, formerly the core of scientific medicine?

How important is pathopsychological knowledge for the physician, and what does it mean to apply the results of basic oriented reserach to clinical practice?

What makes us believe in shared and/or resilient belief? Is it trust in the reliability of specific authorities, of knowledge and rationality, of our own capacities, or of a social system that is usually called 'scientific community'?
